Ghurrix learned the following when casting Legend Lore on The Cenokian Wraith. This information was shared with Tilrinel during the approach to the Cenote in The Underdark.

"Cenokian Wraiths are spectral horrors of former Fae necromancers and sorcerers that corrupt their essence. Through the abuse of necromantic magics, they have defied the God of Death by preventing death even beyond the immortality provided by their Fae nature. With no corporeal form, they were bound to become eternal wanderers, wearing the souls of the deceased on their faces in an image of everlasting horror. Until they are called to collect a particular soul, the Cenokian Wraiths are bound to the Cenotes. There are theories on the symbiosis between the Cenokian Wraith and the Cenote and even the god's lips are silent on the origin of the Cenotes. The Cenokian Wraiths are seen as psychopomps. Attempting to resurrect a person invokes its ire. People living in the wraith's accursed domain learn to fear resurrection and ban components and teaching of resurrections spells lest they call doom unto themselves. In the material world they are the most feared of undead and most who face them go mad with terror before death. The touch of a Cenokian Wraith drains life essence completely, whereby it closes its claws around their victims' organs invoking spasms and death".

The Avatar of Death was encountered by the party during The Drow Arc. After scyring on Akordia in The Underdark, Thoradin's form was raked by spectral claws, tearing away much of his vitality. This was the first interaction the party had with The Cenokian Wraith. During the confrontation with Akordia, the party bore witness to the gargantuan hulking form of The Reaper as it towered over Akordia. The party was then transported to what was believed to be either The Fade or Ashara, wherein they finally did battle with what they believed to be The Reaper. The party quickly realized during the combat that this entity was not the same creature that had attacked Thoradin in The Underdark, as it's blows were able to be withstood by several members of the party. Eventually, the façade of The Reaper dropped to reveal the face of Yestris, whom the original party encountered in the abandoned Warsworn keep in The Plains of Odarth.
